Introduction

These terms and conditions set out the basis on which services are provided by our private Occupational Therapy practice. By engaging with our services, you agree to be bound by these terms.

Appointments

Online assessments 

All appointments must be booked in advance and will be confirmed by email or telephone. If you need to cancel or reschedule, please provide at least 48hours notice. Failure to do so may result in a cancellation fee being charged. Re-booking an appointment - no refund. More than 48hours 100% refunded. Less than 48hours notice 100% will be charged.

Occasionally in unavoidable circumstances we may have to rearrange an assessment at short notice and we reserve the right to do this without incurring liability. Please make sure that we have an up to date mobile number and email address for us to contact you in such circumstances.

Cancellations on the day - if the therapist has not yet travelled, travel time and mileage will not be charged. If the therapist is already travelling at the time of cancellation, travel time and mileage will be charged in part.

Fees and Payment

Fees for services are payable at the time of confirming the appointment.

For example, Functional and work place assessments are booked for 2 hours. The remainder is paid prior to receiving the report ( see report section).

Travel time if appropriate. The first 20 minutes of travel expenses are included in the cost. After this travel is charged at 50% of the hourly rate plus 60p per mile.

There is a 14 day cooling off period, where you can be refunded 100% of the cost if you change your mind about booking.

Payment by bank transfer only. Late payments will result in delays to receiving a service and /or report and incur 10% additional charges .

Reports

Reports are bespoke and as a result can take 2-4 hours to complete. A quote for the complexity of the report will be provided prior to booking. Payment for the report will need to be made prior to its release. Reports securely emailed to you 5 days after the assessment.

Amendments - one change is permitted as long as this is a true representation of what was observed.

Health and Safety

We take health and safety extremely seriously and are committed to maintaining a safe working environment for both clients and therapists. When services are provided on-line or in a client’s home, there are strict safeguarding protocols to adhere to to ensure both parties remain safe and well throughout.

For on-line services, recordings of the session are offered to safeguard both client and therapist. See 'why on-line' section for more information. Emergency information will be obtained if in the event the client has an at home emergency that needs actioning. A safe and secure place for the service user to disclose personal details is guaranteed from the therapist end. 

For home visits, this environment is considered the Occupational Therapist’s place of work, so clients are therefore responsible for ensuring that the environment is safe, appropriate, and free from hazards that could place the therapist at risk. This includes managing pets and ensuring that other household members or visitors do not pose a risk to the therapist’s safety or wellbeing. Smoking is not permitted during sessions.

Both services, if there is any form of verbal abuse, threatening or aggressive behaviour from clients, relatives, or others present, this will not be tolerated. If the therapist feels that their safety or wellbeing is compromised at any time, the session will be terminated immediately and future services will be withdrawn.

Confidentiality

All information shared during sessions is treated as strictly confidential and will not be disclosed to third parties without your consent, except where required by law or for safeguarding purposes.

Consent

Written consent is required before commencing any assessment or intervention. Confirming an appointment is consenting to assessment / intervention. You have the right to withdraw consent at any time, although this may affect the provision of services.

Data Protection

We comply with the General Data Protection Regulation (GDPR) and all relevant data protection legislation. Registered as a data controller with the ICO. Your personal information will be stored securely and used only for purposes related to your therapy and practice administration. Professional Standards

The therapist is registered with the appropriate professional bodies and adheres to the standards set by the Health and Care Professions Council (HCPC), the Royal College of Occupational Therapists and British Associations of Occupational therapists. Who also support Professional insurance.

The Equality Act 2010

Please advise of any reasonable adjustments that you require prior to the assessment, including any communication needs via telephone or email.

Complaints

If you wish to raise a concern or make a complaint, please contact us directly in writing. We will endeavour to resolve any issues promptly and fairly.

Liability

Whilst every effort is made to provide safe and effective therapy, the practice cannot accept responsibility for any injury or loss arising from participation in therapy sessions, except where negligence is proven.

Changes to Terms

We reserve the right to update these terms and conditions as required. Any changes will be communicated to clients in advance.
